The clinicians of Boca Behavioral Health each have their own fee schedules based on years of experience.  Some clinicians accept insurance as in-network providers, others do not.

We will submit claims to the insurance company, when appropriate or provide a Super Bill for you to submit to your insurer.

In Network Insurance:

We participate with most United Healthcare/Optum, Oscar, UMR, Medicare, Aetna and Cigna as “in-network” providers. We are happy to file claims on your behalf. You will only be responsible for your co-pay and/or deductible.

 

Out of Network Insurance:

You may choose to use your “out of network” plan benefits. You will be responsible to pay your fee at the time of service. We will provide you with a "SuperBill" to submit to your insurance company. If you have any questions regarding insurance, please contact your insurance provider.

* If you are not able to keep your appointment, please contact the office to cancel and/or reschedule. Your appointment time has been held specifically for you. If you do not attend your appointment, insurance cannot be billed and your therapist will not be paid for her/his time. If you miss your appointment, without 24 hours notice, a “no show” fee will be charged the amount that is equal to what your therapist would've received, if you had attended the appointment.
